NOTICE REGARDING 1095-C FORMS

In compliance with the Affordable Care Act (ACA) reporting requirements, we are informing you that Form 1095-C, Employer-Provided Health Insurance Offer and Coverage, will no longer be automatically distributed, as a result of the Employer Reporting Improvement Act and the Paperwork Burden Reduction Act. This form was no longer deemed necessary for tax payers and is now only available upon request. If you would like a copy of your Form 1095-C, please submit your request to Human Resources, attn: Marisa Draper, in writing. Please include your full legal name and when making your request. We will provide the requested form within 30 days of receiving your request.
